Case Screening

$250

per letter

All cases begin with a required Case Screening, priced at $250. The purpose of screening is to determine whether a spine-related nexus opinion can be medically supported before a full nexus letter is undertaken. Screening includes a preliminary review of relevant medical and service records, an assessment of service-connection plausibility, and identification of major evidentiary gaps or fatal flaws. Each case is either accepted or declined following screening. The screening fee is non-refundable, does not guarantee acceptance, and is credited toward the cost of a nexus letter if the case is accepted.

Comprehensive Nexus Letter

$750

per letter

If a case is accepted, a Comprehensive Spine Nexus Letter is offered at a flat fee of $750 per spinal region. This fee includes full medical record review, clinical analysis, consideration of accepted medical literature where appropriate, and preparation of a formal written medical nexus opinion using VA-standard language such as “at least as likely as not.” Each nexus letter addresses a single spinal region, including the cervical, thoracic, lumbar, or lumbosacral spine. Multiple diagnoses affecting the same spinal region, such as degenerative disc disease, radiculopathy, or spinal stenosis, are typically addressed within a single nexus letter when medically appropriate. In cases involving multiple spinal regions, each region generally requires a separate nexus letter due to differences in anatomy, biomechanics, and service-connection analysis. Each additional spinal region is priced at $750, with the final determination made during the case screening process.

Addendum Opinion

$250

per letter

A Supplemental or Addendum Opinion is available for $250 and is offered only after completion of an initial nexus letter. Supplemental opinions are limited in scope and may include clarification requested by the VA, response to a VA examiner’s opinion, or additional rationale in support of an appeal. Standalone addendum opinions are not offered without an existing nexus letter authored by Spine Nexus Group.

Standards of Practice

Spine Nexus Group does not offer guaranteed outcomes, expedited or rush services, discounted or bundled pricing, per-diagnosis pricing, unlimited revisions, or nexus letters for non-spinal conditions. Cases that cannot be medically supported are declined in order to maintain medical integrity and defensibility.

Spine Nexus Group accepts a limited number of cases and prioritizes medical rigor, defensible reasoning, and ethical standards over volume.
